Ohio State University has created three useful tools to help in overall giving decisions: Ways to Give, Planned Giving, and Matching Gifts.
By check:
1. Make your check payable to “The Ohio State University Department of Family Medicine."
2. Enter the number of the Endowment Fund or Development Fund in the memo line of your check.
3. Please mail your check to:Attn: Tonya Van Fossen

Online:
1. Go to descriptions of Department of Family Medicine Endowment Funds or Develeopment Funds.
2. Select the fund that you wish to gift and click on the link “Make an online donation to this fund."
3. Click the “Add to Gift List” button.
4. Enter your gift amount, click the “Update Total” button, then click the “Complete Your Gift” button.
5. Verify your gift amount, indicate if this gift is in honor or memory of someone, and then click “Submit | Go to Step 2."
6. Enter your personal information as required.
7. Enter credit card information. Acceptable cards include Visa, MasterCard, American Express, and Discover. Click "Submit."
8. You will receive an e-mail confirmation that your gift was received.
9. Repeat 1-8 if you desire to give to one or more additional funds.
